Emergency Department Care Co-ordinator - Mental Health and AOD Hub role at St Vincent's Health Australia. Part Time, Fixed Term until Sep 2026 (8 hours per week). Location: Fitzroy, Melbourne.
Your Contribution
Nursing qualification or Allied health qualification relevant to the cohort and minimum 2 years' experience in relevant acute setting.
Current AHPRA registration where applicable / and / or eligible for membership of professional association.
Current Victorian Driver's License.
About The Role
This care coordinator role will be embedded into the Mental Health AOD Hub team and report directly to the Team leaders. The role will have strong links to the existing ED care coordination team, ALERT. Through screening and referral in the ED the care coordinator is responsible for the delivery of timely and comprehensive assessment and care coordination of patients with complex needs to facilitate patient centred care, effective management of the ED episode of care and support discharge planning and support into the community. Assessment and care planning are achieved in collaboration with patients, their carers, and members of the MH&AOD Hub team, ALERT and the Emergency Department Clinical team. Classifications for this position will be Allied Health Grade 2 or Registered Nurse Grade 4A.
When not engaged with MH&AOD Hub patients, this role will take on similar duties within the ALERT team.
Responsibilities
Respond and screen eligible clients for the MH&AOD Hub
Provide clinical response to eligible MH&AOD Hub patients
Provide appropriate discipline-specific clinical management, discharge planning and community liaison for all relevant patients in collaboration with team members within the Hub
Coordinate admissions/nominations to inpatient and outreach services
Advocate to ensure safe and coordinated client care
Consult and follow up with community teams to ensure continuity of care
Complete documentation and maintain records in line with SVHM policies and regulatory requirements
Participate in MH&AOD Hub interface processes, service development, evaluation and PD activities
Develop knowledge of local resources to facilitate referrals and service development
ALERT Role
Early identification of ALERT patients in the Emergency Department
Facilitate clinical management, discharge planning and community liaison for ALERT cohort
Incumbent Obligations

Provide information in accessible formats; support consumers in decision-making; inform about rights and how to provide feedback.
Health and Safety
Protect health and safety; complete Fire/Emergency Training annually; complete Workplace Culture and Equity Training annually; attend hospital orientation within 3 months.
Essential Requirements
Nursing qualification or Allied health qualification relevant to the cohort and minimum 2 years' experience in relevant acute setting.
Current AHPRA registration or eligibility for professional association membership.
Current Victorian Driver's Licence.

Specialised skills in Substance Use, Homelessness, Mental Health
Experience prioritizing patients and hospital demands
Knowledge of care coordination frameworks; ability to work autonomously and in an interdisciplinary team
Strong clinical documentation and communication skills
Ability to collaborate with service providers for best outcomes
Experience in program development, implementation and evaluation
Strong time management, planning and prioritization
Participation in roster including weekends and public holidays
Awareness of health reform initiatives
Other Non-Essential Requirements
Specialised skills in aged care or chronic disease, family violence, Aboriginal health
Postgraduate qualifications in related field
