Young Client Needs Assessment Officer required for a permanent NFP opportunity

**Your new company**

We are working with an established not-for-profit organisation who are looking for an experienced Client Needs Assessment Officer to join their growing team on a permanent basis. The Young Client Needs Assessment Officer is responsible for working with various teams in the organisation to plan, develop and evaluate programs and services for families and young clients across WA.

**Your new role**
- Provide support to young clients and their families working within the PHN Primary Mental Health Stepped Care Model
- Intake, assessment and planning sessions with young clients face-to-face and virtually via Microsoft Teams, referring young clients to alternative internal/external services where required
- Advocate on behalf of the client adhering to child safe frameworks - National Principles Of Child Safe Organisations, Department Of Health Child Wellbeing Guide, Rights Of The Child and WA Youth Suicide Prevention Strategy
- Complete case documentation and data entry within best practice policy and procedures, adhering to service KPI requirements
- Participate in individual Clinical Supervision, Case Load and meetings as required
- Work in collaboration with external agencies, networks and community

**What you'll need to succeed**
- Tertiary qualification in Social Work or related field such as Mental Health or Youth Work
- Previous experience in a similar role or experience in formal case management of clients with complex needs
- Previous experience working with vulnerable and disadvantaged children and families
- Ability to travel across Perth Metro to meet clients in their local community
- Ability to develop effective relationships with clients and external stakeholders
- Confidence in providing advisory services, conducting assessments and creating plans
- Demonstrated understanding of the WA support services and education systems available
- C class Drivers Licence, National Police Clearance valid within the last 3 months and ability to obtain a Working With Children's Check
