We are now looking for a Chronic Care Manager to lead a dedicated team in delivering coordinated care and integrated health services across multiple community health programs.Why Join Us?

This involves embedding data-informed renal care planning and risk reduction strategies into Sunrise's PHC model to address high rates of kidney disease in remote communities.This role suits an experienced clinician and manager with expertise in chronic condition care and a commitment to improving health outcomes for Aboriginal people through coordinated, culturally appropriate services.Key Responsibilities:Lead and oversee chronic disease programs and multidisciplinary staff.Ensure evidence-based care planning and quality improvement.Coordinate specialist outreach and partnerships.Manage data, reporting, and compliance requirements.Support recruitment, performance, and clinical development.Represent Sunrise in regional governance and advisory forums.What you'll need to succeed:Minimum 5 years in chronic disease management and/or regional health program deliveryClinical supervisory or leadership experience in a remote Aboriginal health contextDemonstrated understanding of chronic conditions and their social determinantsAHPRA registration as Registered Nurse, Allied Health Professional, or equivalentNon adverse National Criminal History CheckCurrent OCHRE clearanceAcceptable vaccination status to work with vulnerable people in the NTRight to work in Australia.The Benefits:6 weeks annual leave, 18% leave loading15 days personal leavePaid study leave available for professional developmentSalary packaging options that increase your take-home pay significantly, per FBT yearWorking alongside a dedicated and supportive health teamSunrise Health Services is committed to fostering a diverse and inclusive workplace.
