Clinical Coordinators play a vital role in patient care
Alfred Health is a leader in health care delivery, improvement, research and education. We provide a range of services to people living in the inner southeast suburbs of Melbourne, from ambulatory to inpatient and home and community-based services.
- We are seeking Clinical Coordinators to fill this key position
- The successful candidates will be responsible for assessing, coordinating and managing patient admissions under a care coordination model
- This involves liaising with multidisciplinary teams and external stakeholders
- The Clinical Coordinators will also promote our service within the organisation and to external stakeholders
Key Responsibilities:
- Auditing and analysing current care practices to identify areas for enhancement
- Developing and implementing new policies and procedures to improve patient outcomes
- Providing education and training to staff on best practice care techniques
- Working collaboratively with multidisciplinary teams to achieve optimal patient outcomes
Requirements:
- Registered as a Registered Nurse with the Nursing and Midwifery Board of Australia via AHPRA
- Experience within the acute and/or ambulatory/community setting is highly desirable
- A current Victorian Driver's Licence
