**About the Role**:
The Assessment and Care Coordination Officer is required to respond to incoming referrals and provide assertive outreach to engage with and support community members who are homeless, at risk of homelessness or experiencing vulnerability to access appropriate and sustainable housing and/or services
**To be successful in this role, you will have**:
- Tertiary qualification in a health, allied health or social science discipline.
- Minimum two years' experience working with people who are homeless, at risk of homelessness and/or experiencing vulnerability is highly desirable.
- Experience within the mental health sector would be well regarded.
- Assessment, care plan development and experience in accessing referral pathways.
- Well-developed written and verbal communication skills including an ability to communicate with people from diverse backgrounds.
- An ability to identify client needs and respond appropriately in an empathetic, objective, respectful and professional manner.
- Ability to gain trust and build positive relationships with clients and service agencies.
**Mandatory pre-employment requirements**:
- Be willing to undergo and complete a satisfactory criminal history check.
- Provide or obtain an employment-based Working with Children check.
