Established in 1982, Alzheimer's WA provides care, support, awareness and information to assist people living with dementia, as well as their families and carers.

Our services include advocacy, education, partnerships, training, consultancy and holistic person-centred care programs, based on a philosophy of understanding the lived experiences of those living with dementia. We provide CHSP, HCP and NDIS packages and day/overnight respite in our four homes based in Perth, Mandurah and Albany.

**Position Summary**:
Alzheimer's WA is seeking an experienced and passionate consultant to work in our Early Intervention team on a full-time basis at our Subiaco office.

The role of the Early Intervention Consultant, which is a CHSP-funded position, is to provide psychosocial support to people living with early-stage dementia and their primary support person. This support will include facilitating service navigation, performing initial eligibility and needs assessment, and delivering a short-term structured information program to the client and carer. An expectation of the role is the ability to travel to different suburbs to deliver support utilising an Alzheimer's WA fleet vehicle. This role also includes developing and maintaining internal and external relationships and networks, supporting the team and Managers to achieve the organisation's strategic goals.

**Duties of this position include, but are not limited to**:

- performing an initial assessment to establish eligibility and to understand the needs of clients and primary support person;
- providing an appropriate level of support to people living with early-stage dementia and their primary support person through the Early Intervention program and in particular the Adjusting to Change program;
- providing relevant information to facilitate effective service navigation for people living with early-stage dementia and their primary support person;
- contacting clients and carers in a timely manner upon receiving the referral;
- to facilitate peer support groups as part of the Adjusting to Change program;
- empowering and supporting clients to advocate for themselves;
- providing information about other relevant supports and services, facilitates clients to transition into these services as needed; and
- ensuring appropriate program delivery with respect to budgets, funding, and contracts with an emphasis on program development and maintaining standards.

**Essential Criteria**:

- a degree in social work, psychology, or equivalent;
- knowledge of dementia and its impact on the family as well as the individual;
- a minimum of 3 years' experience in the delivery of a community based service or within a health related field;
- ability to work in a small team, lead, manage multiple projects and work with a high level of autonomy;
- strong written and verbal communication skills and interpersonal skills including the capacity to problem solve and negotiate with a diverse range of stakeholders;
- sound knowledge and skill in using MS office suite;
- NDIS Check (or willing to obtain);
- Current WA Driver's Licence;
- Unrestricted working rights in Australia; and
- COVID-19 vaccination certificate (3 doses).

**Desirable Criteria**:

- previous experience in healthcare or aged care;
- a current, valid WA Driver's Licence;
- unlimited working rights in Australia; and
- Licensed Motor Vehicle.
