Responsibilities include:Conduct assessments and develop tailored physiotherapy treatment plans for clients in their homes via home care packages/NDISCollaborate with other health professionals for multidisciplinary careEducate clients and families to promote healthy livingLiaise with service providers for effective communicationWork autonomously, maintaining records and patient reportsSupport from our home care team will help schedule appointments and plan routes.
