The position is casual (minimum 24hrs/wk) and ongoing with some flexibility in hours up to full-time.

Mandatory Qualifications are required for this role

1. Cert III, IV in Disability or Aged Care Services

2. National/International Police Check

3. Working With Children Check

4. NDIS Screening Check

5. First Aid Certificate (incl. resuscitation)

6. Valid Driver's License and Access to an insured vehicle.

7. Proof of Covid-19 Vaccination status.

8. Introduction to the NDIS training module certificate.

**Position Title: NDIS Support Coordinator Team Leader / Supervisor**

**The National Disability Insurance Agency (NDIA) defines Support Coordination as**:'Assistance to strengthen participants' abilities to coordinate and implement supports and participate more fully in the community.' It can include initial assistance with linking participants with the right providers to meet their needs, assistance to source providers, coordinating a range of supports both funded and mainstream and building on informal supports, resolving points of crisis, parenting training, and developing participant resilience in their own network and community.'

**Reports to: Natallie (Director)**

**PRINCIPAL ROLES AND ACCOUNTABILITIES**:
This position will provide direct support and supervision for a group of Support Coordinators in addition to engaging in a small number of direct Support Coordination of complex cases. Support Coordinators are responsible for providing independent support to help NDIS participants and their families understand, build skills, and implement their NDIS plan.

**The direct practice of this position includes**:
**Coordination of Supports**:
Assistance to strengthen participant's abilities to connect to and coordinate informal, mainstream, and funded supports in a complex service delivery environment. This includes resolving points of crisis, developing capacity and resilience in a participant's network, and coordinating supports from a range of sources.

**Specialist Support Coordination**:
This is a higher level of support coordination. It is for people whose situations are more complex and who need specialist support. A specialist Support Coordinator will assist participants and their families to manage challenges in their support environment and ensure consistent delivery of service.

The Support Coordinator Team Leader / Supervisor will work as part of a team to achieve the strategic goals of the organisation.

**Key responsibilities** of the Support Coordinator Team Leader / Supervisor are to:

- Supervise and support a group of Support Coordinators, to ensure the staff's optimum development and success in the role.
- Work with Company Director/Support Coordination Manager, (Natallie) to provide leadership in Support Coordination.

**DUTIES AND RESPONSIBILITIES**
- Provide leadership, regular supervision and support for a group of Support Coordinators
- Team leader is expected to work 24 - 38 hrs per week.
- Participate in recruitment of Support Coordinators, interviews, staff selection, referee checks and other required checks.
- Provide orientation and training to new Support Coordinators and ensure staff on-boarding and HR functions are completed.
- Participate in an On-Call system for the organisation (negotiable).
- Manage staff including staff performance management, appraisals, complaints, and issue resolution.
- Cover essential staff work when Support Coordinators are on individual leave.
- As part of the Support Coordination leadership team, provide expert advice in one or more key areas of the service and NDIS.
- Internal monthly meetings with the Support Coordinators.
- Monitor support coordination staff ensuring:

- services are provided within the time parameters of the participants' plan.
- adherence to timely documentation and billing procedures.
- NDIS timelines and reporting requirements are met
- Oversight of billable hours within the participants' plans and liaison with support coordination staff and finance to troubleshoot any problems.
- Take a leadership role in supporting and directing staff to meet individual and team billable hours and KPI targets.
- Daily supervision of direct reports.
- Drive the promotion and growth of the NDIS service provision
- Be responsible for team outcomes, ensuring targets and funding requirements are met, and good external relationships are maintained.
- Ensure compliance with all relevant legislation, funding guidelines, service standards and contractual obligations.

**To do a great job, you'll need**:

- Minimum Certificate IV in Community Services, Minimum Certificate IV in Case Management, or other relevant Certificate IV.
