Our Operational Communications division is responsible for coordinating and optimising state-wide emergency and non-emergency ambulance response, as well as the provision of patient care through Australia's largest ambulance triage directorate.
About the role
Ambulance Victoria currently have an alluring opportunity for the role of Patient Management Lead to join our Triage Services directorate. Reporting to the Manager Clinical Triage as a member of the leadership team, this position is primarily responsible for coordinating and overseeing the Patient Management team. The Patient Management Lead will provide expert advice and clinical support in relation to complex patient management plans to improve integrated service provision, and provide better access and coordination of care for patients.
With a key focus on leading day of operations activities for the Patient Management team. Your responsibilities will include, but are not limited to:

- Manage, oversee and contribute to the development and delivery of frequent and complex patient management plans and activities
- Lead day to day operations as a key contact point for enquiries, information sharing and authoritative advice
- Participate in multidisciplinary reviews with internal and external stakeholders, to ensure ongoing alignment with state-wide complex care pathways
- Lead and model Best Care to improve AV's care and response to vulnerable patients with complex needs, ensuring excellence in patient care standards
- Ensure high quality documentation standards are met and reflect the current patient care arrangements
- Ensure outcomes and processes are routinely monitored, reviewed and evaluated to ensure they meet current legislation and best practice,
- Provide strong and effective leadership, management and support to direct reports by fostering a culture of professional development, leadership and continuous improvement to ensure goals are achieved and to drive superior business outcomes
- During periods of escalating demand may be required to provide medical or other assistance to patients in an emergency, in line with AVs policy and procedures

About you
Suited to highly motivated individual who is passionate about improving care responses for patients with complex social and health care needs, the successful applicant will possess:

- Significant experience and a thorough knowledge of frequent and complex patient management practices working in a complex organisation with diverse business requirements in the Health or Emergency Services field
- Current AHPRA Paramedic or Division 1 Nurse registration
- Demonstrated management and proven leadership and coaching skills
- Contemporary clinical knowledge and skills
- Experience in the development, implementation and evaluation of process improvement
- Experience developing networks and working relationships with key internal and external stakeholders and within external/community organisations
- Integrity and judgement to ensure matters of a confidential or sensitive nature are handled appropriately

All applicants must have unrestricted working rights in Australia.
The commencement salary for these positions is $140,820 annum, pro-rata in alignment with the AVMA-Schedule B award, plus 10.5% superannuation. All staff are required to complete 8 weeks induction and training at full time hours prior to being eligible for a part time (pro-rata) contract.
