The Hospital Outreach Post
- suicidal Engagement (HOPE) program is a core component of _Victoria's suicide prevention framework 2016-25 _and is a psychosocial and clinical support service that delivers responsive outreach to individuals following suicide attempt or are considered at significant risk of suicide with serious planning or intent and/or repeated intentional self-harm.
**About the role**
The HOPE teams support individuals and their personal support networks (family, friends and other carers) for up to three months helping them to identify and build protective factors against suicide. This role provides operational oversight of the HOPE team.
**Key Responsibilities**
- The Team Leader is the pivotal link between the senior leadership team and the HOPE Team and is expected to provide high level operational leadership and ensure the service provides a high quality and accessible service to individuals and the community of the Grampians region.
- The Team Leader will also work positively towards future developments from within Grampians Health and implementation of recommendations from the Mental Health Royal Commission.
**Skills and experience**
- Current registration as a Registered Nurse Division 1, Occupational Therapist or Psychologist with the Australian Health Practitioner Regulation Agency (AHPRA) or
- Approved degree in Social Work and eligibility for membership of the Australian Association of Social Workers.
- Postgraduate qualifications in the area of specialty and/or management will be highly regarded
- Extensive clinical practice reflects an advanced level of knowledge, particularly with complex presentations and management of static and dynamic risk.
- Demonstrated experience in the supervision of others, preferably in an environment of Community Mental Health and service development.
