Central Bayside Community Health Services (CBCHS) - Social Support Group team has an exciting opportunities for compassionate and motivated Lifestyle Support Worker.
- We have
- **Part Time **roles available, our programs run between
- **8.30am and 4.30pm, Monday to Friday** with no late nights or weekend work required. We support people with a range of abilities, from independent, and complex support needs.
- We offer a variety of choice with interesting centre and outing based group activities, with foundations in Recreation and Leisure. We take time to understand our clients goals and aspirations, and pro-actively develop positive support strategies to enhance their quality of life.

The Lifestyle Support Worker is a rewarding and unique opportunity to work in a group program. This role is supporting the Group Supervisor in both delivery and coordination of the Social Support Group activities that develop, maintain and support s
- ocial interaction and independent living (SSG) for older persons and older persons experiencing cognitive changes. This role also provides valuable interactions, care and support to participants with the ability to build a real connection with clients.**Responsibilities**

Program Delivery- Assist the group Supervisor in planning and organising a program that includes socialisation, recreation and crafts.
- Provide personal care to clients i.e. meal and toileting assistance.
- Assist the Group Supervisor in liaising with clients.
- Assist the Group Supervisor in liaising with carers and families of clients.
- Closely supervise clients to ensure their safety at all times.
- Respond to and be flexible to changes in daily duties as required.
- Assist the Group Supervisor in organising and providing transport for clients to and from the group as well as outings. Drive CBCHS vehicles as required for transporting clients.
- Ensure all emergency and safety procedures are adhered to.

Administrative duties- Maintain appropriate records and reports and adhere to the administrative procedures of the service.
- Maintain relevant records and documentation for transport including Vehicle Daily Checklist.
- Ensure any accidents or incidents are promptly reported and documented in accordance with CBCHS policy and procedures.

**About you**

- Current Level III Aged care or Equivalent.
- Demonstrated working experience within an aged care /community health setting or related areas.
- Understanding and proved commitment to the aged and experience dealing with clients and carers
- Well-developed communication skills both written and verbal, including the ability to liaise with families, carers and other professionals from a diverse range of cultures and backgrounds.
- Demonstrated focus on service excellence with a commitment to delivering high quality outcomes for clients.
- Ability to work flexibly and adapt to changing situations.
- Ability to demonstrate on-going drive and commitment in a challenging environment.
- Demonstrated ability to work well as a part of a team and follow direction as required.
- Good organisational and time management skills.
- Current Victorian Drivers licence.
- Previous experience in person centred planning is desirable.
- Previous experience in development of Behaviour Support Plans is desirable.
- Current First Aid Certificate is desirable.
