**About Us**:
Part of **Lutheran Services**, **Zion Home Care** at **Nundah** supports those to live in their own homes for longer
- to live life to the fullest with a reliable, friendly carer by their side, with whatever support they need.

**About the role**:
The Home Care Coordinator:

- Ensures that care needs are being met through high-quality service delivery
- Supports clients in developing or reviewing their Client Goal/Care Plan
- Coordinates and monitorsthe delivery of services, maintaining appropriate communication with carers and family members
- Mentors home care workers in the community for their benefit, and that of thier clients.

**This is a permanent, part time role - 3 days per week**

- The ability to make sound judgements and develop high-level Client Goal plans based on your knowledge of Home and Community Care
- Enjoy working with a team to deliver services
