Mercy Community Services invites dedicated individuals to join our team as a Home Care Coordinator for the Queensland Community Support Scheme (QCSS). QCSS is a lifeline for those who, while not eligible for NDIS, experience disabilities, chronic illnesses, mental health conditions, or other circumstances that hinder their ability to live independently within the community. As a Home Care Coordinator, you'll partner with clients & their families to support & ensure that their needs are met.

**About your role**:
Typically the Home Care Coordinator role will involve:

- Lead and supervise a team of community support workers including completing performance reviews and identifying areas of development
- Respond and assess referrals in the Service Provider portal
- Liaise with external case managers, GP's, and other allied health professionals for complex consumer needs
- Monitor service delivery and provide statistical data
- Negotiate, mediate, and resolve conflict as well as responding to emergency situations.

**About you**:

- Certificate IV in Aged Care Services or Community Support or the equivalent
- Three years' experience working within a Community setting in a similar role with leadership experience
- Knowledge of community supports, inclusive of the Current Aged Care reforms
- Queensland Provisional or above licence
