Nepean Hospital is a teaching hospital that provides tertiary referral services in emergency care, coronary care, diagnostics, gynaecology, paediatric, neonatal intensive care/ICU, maternity, mental health, rehabilitation, and surgery. The Nepean Campus has recently experienced tremendous growth and aims to support communities within and outside of Sydney's western suburbs, providing world-class clinical facilities, services, and care.

The Patient Flow Management team is seeking to engage a Clinical Nurse Consultant in the role of Continuing Care Coordinator. The role provides a specialized nursing consultancy service to patients, carers, and other health care professionals in the separation planning and continuum of care process at Nepean Hospital. This includes a timely facilitation, coordination, and planning of a person-centred, safe transfer of care for patients referred through e-Orders.

In this position, you will serve as a clinical expert who through leadership facilitates evidence-based practice. We are looking for an incumbent who will model our CORE (Collaboration, Openness, Respect and Empowerment) values and exercise autonomy in decision making within scope of practice.

**What you will bring to the role**:

- Current registration with the Australian Health Practitioner Regulation Agency (Ahpra) as a Registered Nurse with a minimum of five years' experience in an acute setting two years clinical experience in specialty
- Demonstrated effective clinical communication using information technology and sound written, verbal and interpersonal skills.
- Demonstrated ability to work effectively and collaboratively as part of the Multidisciplinary Health Care team
- Proven ability to prioritise and organise own workload
- Demonstrated ability to network and liaise with government and non-government service providers
- Consistently demonstrates behaviours that reflect the CORE Values; Collaboration, Openness ,Respect and Empowerment of Nepean Blue Mountains Local Health District.
- Understanding and knowledge of current NSW Ministry of Health Policy Directive Admission to Discharge Care Coordination and how this guides practice
