The role of the Connections Transition Coordinator is to improve continuity of care for patients who are in public correctional centres with drug and alcohol problems, that are being released to the community.
The position works within an assertive linkage model that aims to link patients with relevant health and welfare service providers pre and post release.
Key Responsibilities- Utilize a broad array of contacts, both in the correctional and community environments to assertively link patients into services appropriate to their individual needs.
The successful candidate will have:
- A strong understanding of Drug and Alcohol treatment approaches.
- Experience in managing continuity of care for clients with chronic and or complex health needs.
- Excellent communication and interpersonal skills.
