Job Description
About the roleSeeking a Level 3 Social Worker, who will provide psychosocial assessments and complex discharge planning for two inpatient oncology wards with a large elderly patient population.
Work closely with medical teams to deliver patient-centred care in a supportive, part-time role.
Key responsibilities
Deliver psychosocial assessments for oncology inpatients
Lead complex discharge planning with multidisciplinary teams
Provide support to patients, families, and carers
Skills & experience
Degree in Social Work
Current or eligible AASW registration
Strong skills in discharge planning and assessments
Fully vaccinated for COVID-19 (including booster)
