About the Opportunity

As a Care Partner, you'll be the friendly face and trusted advisor for older South Australians navigating their aged care journey. This customer-focused role puts your interpersonal skills front and centre as you:

  • Build genuine connections while you meet with clients and families, establishing trust and understanding their unique needs and goals
  • Conduct comprehensive assessments to evaluate wellbeing and identify what support will make the biggest difference
  • Manage a client's government funding and link them in with services as they walk through their aged care journey.
  • Develop clear, actionable plans that empower clients to live independently and comfortably
  • Partner with families, healthcare professionals, and our multidisciplinary team to ensure clients receive the right support at the right time
  • Monitor your caseload and adapt plans as circumstances evolve
  • Manage client relationships, provide updates, and maintain strong communication with all stakeholders

This is a dynamic role where no two days are the same. You'll split your time between face-to-face client visits, team collaboration, and case management activities.

About You

Essential Criteria

  • Relevant Tertiary qualifications in the Human or Behavioural Sciences, Allied Health, Aged Care or Community Services Fields
  • Experience in client case management
  • National Police check
  • Driver's licence
  • Any case management experience, placement experience from your studies, or volunteer work with older adults or community services would be an advantage.
