The opportunity
Join our team as a Care Partner and make a meaningful impact by delivering person-centred care that supports independence and wellbeing. You'll be a trusted guide, helping clients navigate their care journey with compassion and expertise. This full-time, permanent role offers a hybrid work arrangement — split your time between home and our supportive office environment at Park Holme or Elizabeth South.

Key Responsibilities
- Conduct strengths-based assessments and create personalised care plans aligned with client needs and contractual obligations
- Provide person-centred case management and coordinate services for clients and carers with moderate to complex needs
- Manage budgets and ensure effective resource use
- Collaborate with health professionals and community organisations
- Maintain accurate records and meet reporting requirements
- Adapt to challenges with resilience and a solutions-focused mindset
About you
You're an experienced professional in aged care (perhaps a Home Care Package Coordinator, Care Partner, Enrolled Nurse, or CHSP Coordinator) ready to take the next step in your career.
You bring:
- Certificate IV in Community Services (or equivalent) and a current First Aid
- Proven experience in case management, service coordination, and care planning
- Strong understanding of SAH budgets and community care compliance
- Excellent communication, decision-making, and time management skills
- Confidence using digital tools (Word, Excel, calendars)
