Our client is a renowned community services provider with a strong presence in the community. They offer a wide range of social welfare services designed to meet the diverse needs of individuals across all age groups, from infants to seniors. These services encompass residential aged care, home aging support, settlement and health services, vocational training, disability support, child care, and an array of other community-based services.
We are currently in search of a motivated and highly skilled individual to join the organization.
Position Summary:
We are looking for a dynamic individual to play a pivotal role in our Home Ageing Services (HAS) Unit, working closely with the Executive Team. The primary responsibility of the position holder is to assist in the development and delivery of high-quality services to seniors dealing with issues related to aging, including dementia, chronic illnesses, and mobility concerns. The incumbent is expected to ensure that all services are in alignment with established service guidelines, organizational policies, Work Health and Safety (WHS) procedures, and actively contribute to continuous improvement initiatives.
Requirements
Main Duties and Responsibilities:
• Provide case management for home care clients, including conducting assessments and creating personalized support plans.
• Actively promote and expand the organization's home care services, taking the lead in introducing these services to potential clients.
• Periodically compile statistical data to facilitate efficient and cost-effective management of Home Care Services. This includes updating client information and maintaining the database.
• Ensure that services comply with all funding requirements, program guidelines, Aged Care Quality Standards, HAS Policies and Procedures, organizational policies and procedures, Restorative Principles, and all other legal requirements applicable to service providers.
• Manage funding applications and oversee projects related to care services. This involves coordinating activities and resources, monitoring project progress, identifying and addressing any issues or challenges, and ensuring timely completion.
• Actively participate in ongoing quality improvement activities.
• Fulfil any other duties as required and directed by the Executive Team of the HAS Unit.
Essential
• Possession of a tertiary qualification in nursing, health, welfare, or a related field.
• Demonstrated experience in Government-funded home aged care programs for a minimum of 2 years.
• Demonstrated knowledge and experience in applying a person-centred approach and consumer-directed care model, with a focus on wellness and reablement.
• Strong ability in the development and implementation of quality improvement measures in accordance with service standards.
• Proficiency in spoken and written English.
• Excellent communication, interpersonal, time management, and computer skills.
• Proven skills and experience in working with primary and allied health professionals to provide services to clients.
• Current First Aid Certificate.
• Valid police check certificate or the ability to obtain one before employment.
• Valid Australian Driver's License with access to a personal vehicle.
