Where you'll be working
The End of Life Care Social Work position plays a vital role within the Maitland Dungog Palliative Care Team, delivering compassionate and specialised support to patients and families facing life-limiting illness. This role is primarily based at the Old Maitland Hospital Campus, allowing close collaboration with medical, nursing, and allied health professionals in a multidisciplinary setting. In addition to its local focus, the position is an integral part of the wider Lower Hunter Social Work Team, contributing to a coordinated and consistent approach to palliative and end-of-life care across the region.

What You'll Be Doing
This Social Work position is a community-based role serving the Maitland and Dungog LGAs, working closely with the Palliative Care team through referrals. The primary focus is to provide comprehensive psychosocial support to clients and their families, including assessments, therapeutic counselling, group work, bereavement support, and connecting clients with relevant services.

The role is supported by the Lower Hunter Head of Social Work and Nursing management, with direct line management provided by the Head of the Social Work Department. Monthly supervision is a mandatory component of the position to ensure ongoing professional guidance. The role is part of the Lower Hunter Social Work Department, which meets monthly to promote collaboration and shared learning. Additionally, professional development is actively encouraged and supported through the Lower Hunter Allied Health Department.

About You
This position requires a social worker who is capable of working independently with minimal direct supervision, demonstrating strong decision-making and time-management skills. The successful candidate must possess comprehensive psychosocial assessment abilities to effectively evaluate and address the complex needs of clients and their families. A current driver's license is essential, as the role involves regular travel across the region to conduct home visits and provide community-based support. Additionally, the successful candidate must have advanced skills in collaborating within a multidisciplinary team, working closely with healthcare professionals to deliver coordinated and holistic care.

Requirements

  • Bachelor in Social Work or equivalent with eligibility for membership with Australian Association of Social Work (AASW)
  • Eligibility to drive in NSW
