This role is primarily based at the Old Maitland Hospital Campus, allowing close collaboration with medical, nursing, and allied health professionals in a multidisciplinary setting. In addition to its local focus, the position is an integral part of the wider Lower Hunter Social Work Team, contributing to a coordinated and consistent approach to palliative and end-of-life care across the region.

What you'll be doing

This Social Work position is a community-based role serving the Maitland and Dungog LGAs, working closely with the Palliative Care team through referrals.
