Embark on a rewarding career and be part of a team where your dedication makes a difference in people's lives.
Working in Aged Care has never been more important or meaningful. Be part of a community aged care service that is embedding evidence-based practice and providing outstanding clinical care.
About The Role:
We're looking for a motivated Grade 2 Speech Pathologist to join the Aged and Community Care (ACC) program at Monash Health. This part-time ongoing role (0.4 EFT / 30.4 hours per fortnight) is part of a collaborative, interdisciplinary service that supports older people to live independently at home and in the community.
You'll work within a dedicated multidisciplinary team to deliver holistic, evidence-informed care that is person-centred, trauma-informed, and culturally sensitive. From conducting swallowing assessments and communication evaluations to providing therapy for clients recovering from a stroke with aphasia or helping someone with dysphagia to eat and drink safely, your interventions will be tailored, practical, and grounded in the latest evidence.
During your day-to-day you will:
- One-on-one therapy sessions - empowering clients with practical tools and strategies that promote their independence
- Leading group programs like Aphasia or Communication Groups, where you'll create an inclusive space for clients to practice and improve their communication skills
- Providing remedial compensatory, educational, and preventative strategies, tailored to client needs
- Collaborating closely with other allied health clinicians and external community-based services to help maximise the clinical outcomes and operational efficiency of the program
- Play a key role in delivering person-centred care - whether through home visits, telehealth, or collaboration with community-based professionals - to empower older people to live well and independently
You'll also help shape service delivery through active collaboration, ensuring our clients receive compassionate, coordinated care that supports their goals and independence.
About Us:
It's an exciting time to be part of the aged care sector, with national reforms placing older Australians at the centre of care. These changes focus on improving quality, safety and choice - ensuring services are responsive, respectful, and person-centred.
At Aged Care Services, we deliver support across the full continuum of Commonwealth-funded aged care - home care, community programs, and residential services.
Our vision is simple yet powerful: to provide exceptional care and experiences that are sustainable, responsive to individual needs and preferences, and that uphold the rights of older people.
The program aims to keep the client independent within their community, proactively promoting health and independence, and self-management whilst assisting with timely interventions to prevent deterioration of the condition, and admission to hospital or residential care.
About You:
You're passionate about supporting older people live well at home and in their communities. You bring a collaborative, organised approach and a strong commitment to delivering high-quality, person-centred care - always with empathy, cultural sensitivity, and respect.
To thrive in this role, you will bring the following:
- A recognised degree in Speech Pathology (Bachelor or Master level), including overseas qualifications assessed by Speech Pathology Australia (SPA) within the past two years
- Eligible for membership as a Certified Practising Speech Pathologist (CPSP) with SPA
- Current Victorian drivers licence
- Current and Satisfactory Aged Care Clearance (Police Check/Aged Care Worker Screen)
- Current and Satisfactory NDIS workers check and NDIS worker orientation certificate of completion
- No current Aged Care Banning Orders in place (as per Register of Banning Orders)
- Registered with State-Wide Equipment Program
In Return:
As a valued member of the Monash Health team, you'll have access to a wide range of benefits designed to support your wellbeing, development, and career progression, including:
You'll be joining the largest public Speech Pathology department in Victoria
Achievable/ manageable KPI with strong focus on delivering evidence based, comprehensive care
