Perth, Western Australia SPECIALIST DISABILITY HOUSING PTY LTD Full time $45,000 - $75,000 per year

Specialist Disability Housing (SDH) is currently looking for highly active Disability Support Workers Female (Casual) with minimum 2 year of experience. We are looking for compassionate, reliable, and experienced disability support workers for our clients.

Key Responsibilities

  • Support participants to work towards achieving their individual goals
  • Support daily living, promoting cooking, housekeeping, hygiene, shopping, Medication administration, community access etc.
  • Support participants in building their confidence.
  • Liaise and collaborate with clients, their community and community & disability support agencies.
  • Monitor and review the participant's progress through the service and against the support plan.
  • Utilize positive behavior strategies
  • Support clients to participate in age-appropriate activities and tasks, including employment, in-home disability support and community access.
  • Work in accordance with the medication administration policy, work health and safety procedures and client rights policies, including privacy and confidentiality.
  • Monitor the safety and well-being of people with disability and report any concerns to the Manager.
  • Ensure completion of appropriate documentation. As required, relevant information on disability support will be recorded in client notes and communication books.
  • Participate in the Lifestyle Service Planning process for individual clients and monitor outcomes.
  • Work with clients, family/ carers, relevant case managers and other professionals to support the client's individual goals.
  • Liaise extensively with other staff to ensure the overall smooth and consistent running of the disability services.
  • Report regularly to the Manager

Skills & Experience

  • Qualification in community/disability/individual support/aged care/ Mental Health and preferably with relevant experience.
  • Experience working with clients with disability & challenging behaviors.
  • Well-developed verbal and written communication skills.
  • Current Driver's License and reliable vehicle.
  • 100-point check ID
  • Valid visa/working rights in Australia

Job Type: Casual

Pay: $35.15 per hour
