WHAT ARE WE OFFERING?
Join the Optimal Ageing Team within the Community and Allied Health Directorate at Country Health Connect, where you'll play a key role in supporting older people to remain healthy, independent, and connected in their own homes.

The Barossa Hills Fleurieu Local Health Network runs from Kangaroo Island to Eudunda, wrapping all the way around our metropolitan areas (this position is in Victor Harbor). WHERE AND HOW YOU WILL WORK
The Fleurieu Peninsula is just one hour for the city of Adelaide. It covers the stunning areas and towns of McLaren Vale, through to Strathalbyn, Goolwa, Victor Harbor and Aldinga Beach where you can spend days off experiencing the wineries and award winning restaurants of the southern vales. Enjoy our stunning coastline from Cape Jervis and Rapid Bay up to Aldinga and Sellicks Beach, whilst only being just over an hour from Adelaide's CBD. Catch a glimpse here.
HOW WILL YOU CONTRIBUTE?
- Coordinate referrals, assessments, and reviews for aged care clients in their homes
- Develop client-directed care plans and ensure timely rostering of support workers
- Supervise and support Community Support Workers to deliver safe and high-quality care
- Liaise with clients, families, staff, and service providers to ensure care meets individual needs

WHAT ELSE DO YOU NEED TO KNOW/WHAT IS ESSENTIAL FOR SUCCESS?

- You will need to be immunised. Please contact the Enquiries Person for this position if you have any queries. Addressing vaccine preventable disease: Occupational assessment, screening and vaccination policy. Further information is in the application for to help guide you.
- You will need to undergo employment screening, what you require is also on the front page of the Role Description.
- You will be required to hold Certificate 3 in Individual Support (Ageing) or equivalent, or willing to work towards same.
