Role & Responsibilities:

  • Manage an appropriate case load effectively through planning, clinical prioritisation, and proactive time management skills

  • Function as an advisor in relation to the provision of Aged Care Services. Ensure the alignment of technologies, methods and practices with the Home Care Standards, Charter and relevant organisational policies and procedures

  • Identify individual consumer's needs resulting in the development and implementation of a holistic therapy plan in consultation with the consumer, carer, relevant stakeholders, and frameworks (internal and external)

  • Ensure allied health and therapy plans are developed based on the goals, strengths and identified needs in consultation with consumers. These are to be reviewed in accordance with policies and frameworks (internal and external)

  • Leading and participating in programs that may enhance the older persons wellbeing, quality of life and preference to remain living in their own home

  • Conduct and complete comprehensive assessments and defined measures, and use specified data collection tools and outcome measures to record and demonstrate changes in function and to ensure consumer outcomes

  • Supports the identification of and management of consumers with chronic and complex care needs

  • Work autonomously, including clinical decisions (without onsite supervision, as appropriate)

About you?

  • Current registration as an Occupational Therapist with the Australian Health Practitioner Regulation Agency (AHPRA)

  • Sound knowledge and practical experience working with people who are ageing from an enablement, restorative and wellbeing perspective, including assessment and the delivery of evidence-based skills

  • Understanding of case coordination and multidisciplinary teamwork

  • Demonstrated ability to lead and promote change and contribute positively to workplace culture

  • Confident and skilled clinician, able to engage and motivate consumers, families, staff and stakeholders

  • Experience and commitment to working with quality and risk management systems
