As our services continue to grow, we are seeking a proactive, compassionate, and experienced Occupational Therapist (Community Care Partner) to join our dedicated team. About the Role

As an Occupational Therapist (Community Care Partner), you will play a pivotal role in ensuring client-centred, outcomes-focused care delivery. You will provide occupational therapy assessments and interventions while also acting as a trusted care partner—supporting clients, families, and support workers to deliver seamless, coordinated services under Support at Home (SaH) and NDIS programs.

Key ResponsibilitiesOccupational Therapy – Community-Based Practice

  • Conduct comprehensive OT assessments in clients' homes (functional capacity, mobility, safety, and independence).
  • Develop, implement, and review individualised intervention plans aligned with client goals.
  • Recommend and coordinate assistive technology, home modifications, and equipment.
  • Provide education and strategies to clients, families, and support workers to promote independence and safety.
  • Complete clinical documentation and reports in line with SaH and NDIS requirements.

Care Partner / Case Management

  • Act as a key point of contact for allocated clients and families.
  • Support clients to understand and achieve their goals under Support at Home (SaH) and/or NDIS plans.
  • Coordinate services in collaboration with support workers, care managers, and external stakeholders.
  • Review and adjust care plans based on changing needs, feedback, and clinical outcomes.
  • Build trusted, long-term relationships that enhance client satisfaction and continuity of care.

Care Coordination & Collaboration

  • Work closely with scheduling and care coordination teams to ensure services align with assessed needs.
  • Provide clinical guidance to support workers to ensure safe and effective care delivery.
  • Participate in case discussions, service reviews, and multidisciplinary collaboration.
  • Support risk identification, incident follow-up, and continuous improvement activities.

Administrative & Compliance Responsibilities

  • Maintain accurate, up-to-date client records in care management systems.
  • Ensure services align with SaH funding guidelines, service agreements, and safety standards.
  • Uphold privacy, confidentiality, and professional boundaries at all times.
  • Contribute to quality assurance, reporting, and documentation requirements.

Essential Skills & Attributes

  • Qualified Occupational Therapist with relevant registration.
  • Experience working in community, aged care, disability, or home care settings.
  • Strong understanding of Support at Home (SaH) and/or NDIS frameworks.
  • Warm, empathetic, and confident communication style.
  • Excellent time management and ability to manage a varied caseload.
  • Strong problem-solving skills and client-first mindset.
  • Confidence using Microsoft Office and care management systems.
  • Ability to work independently while collaborating within a supportive team.

Essential Requirements

  • Current AHPRA registration as an Occupational Therapist (unrestricted).
  • National Police Check (or willingness to obtain).
  • Working With Children Check (where applicable).
  • Evidence of right to work in Australia.
  • Valid driver's licence and access to a reliable vehicle for community visits.
  • Previous experience in community-based OT or care coordination (preferred).
