Footprints Community is a well-regarded not-for-profit provider of community-based services, working in the community for over 30 years. We specialise in working with older people, those that experience disability, mental illness, as well as those who are at risk of homelessness.
About the role
Mental Health Practitioner (Identified) at the Medicare Mental Health Centre play a vital role focused on providing high-quality, person-led mental health services. They will work within a multidisciplinary team to deliver therapeutic interventions, wellbeing coaching and support to individuals and or their families, experiencing a range of mental health challenges.
This role will be responsible for providing short to medium term psychological therapeutic support, service navigation and crisis intervention to all individuals, families, and groups with a special interest in First Nations practice and required to work in collaboration with the Practice Lead and Footprints Manager First Nations. This role is vital in supporting the mental and emotional well-being of our clients, ensuring they receive the care and support needed to improve their quality of life.
Key Responsibilities
- Approach practice with First Nations (and others) clients in a culturally safe and appropriate way.
- Work in collaboration with the Footprints Manager First Nations to ensure cultural capability in practice.
- Meet with local First Nations service providers and stakeholders when appropriate.
- Work collaboratively with non-identified team members who are working with First Nations clients to offer guidance and cultural support to develop their skills and cultural practices.
- Conduct biopsychosocial assessments using the Initial Assessment & Referral Decision Support Tool to understand individuals needs.
- Perform risk assessments and develop thorough and practical risk management plans and safety planning
- Work collaboratively with a team of lived experience and mental health professionals, including psychologists, social workers, and occupational therapists.
- Participate in case conferences and team meetings to discuss guest progress and develop integrated support strategies.
- Educate guests and their families about mental health wellbeing, available support and therapeutic approaches.
- Maintain accurate and up-to-date person led records in compliance with organisational policies and confidentiality requirements.
- Build and maintain relationships with community organisations and referral sources.
- To perform this role, it is essential that the person identifies as an Aboriginal and Torres Strait Islander person. It is therefore a genuine occupational requirement under section 25 of the Anti-Discrimination Act 1991 (QLD)
- A recognised tertiary qualification in Psychology, Social Work, Counselling, Occupational Therapy or a related field.
- Current registration with relevant professional bodies (e.g., AHPRA, ACA, PACFA).
