About the Opportunity

As the Clinical Nurse Consultant (CNC) Grade 2 – Virtual Diabetes Care, you'll be at the forefront of innovation—designing, implementing, and leading a nurse-led telehealth model that transforms diabetes management across the Murrumbidgee Local Health District.

You'll harness the power of digital tools such as the NSW eHealth Diabetes Management Dashboard, live glucose management worklists, and District DKA data to identify, monitor, and support high-risk patients who need timely, specialist-informed intervention.

About You:

You are a forward-thinking clinical leader with a passion for innovation in healthcare delivery.

You combine expert knowledge in diabetes management with a deep understanding of nursing practice, virtual care, and rural health challenges.
