About the Role
We are seeking a highly experienced, self-driven, and respectful Disability Support Worker to support a participant in Greenvale VIC with complex physical, neurological, and psychosocial needs.
This is not an entry-level role. It requires a professional who can:
- Deliver high-intensity personal care with skill and attention to detail.
- Manage challenging behaviours calmly using trauma-informed approaches.
- Liaise confidently with doctors, hospitals, and allied health teams.
- Support meaningful community access in a bright, vibrant, and proactive way.
- Maintain clear, detailed documentation that meets NDIS compliance standards.

Key Responsibilities
Personal & Health Care
- Reposition participant every 2–4 hours to prevent pressure injuries.
- Assist with hygiene, dressing, grooming, and continence care.
- Administer and monitor medications as per the care plan.
- Follow strict infection control and manual handling protocols.
- Monitor pain, wounds, and health status; escalate concerns promptly.
- Provide safe hoist transfers.
Behavioural & Emotional Regulation
- Implement behaviour support strategies consistently and professionally.
- De-escalate incidents using trauma-informed care.
- Communicate calmly and respectfully, even under pressure.
- Promote participant choice, autonomy, and dignity.
Daily Living & Community Participation
- Support with meal preparation and feeding where required.
- Assist with light domestic duties (laundry, cleaning, tidying).
- Accompany participant to medical appointments and community outings using accessible transport.
- Provide positive, engaging, and proactive support during community and social activities.
- Encourage the participant to work towards NDIS goals and meaningful daily routines.
Documentation & Reporting
- Maintain clear, factual, and detailed progress notes.
- Complete incident reports, risk escalations, and care updates in line with NDIS Practice Standards.
- Collaborate effectively with coordinators and allied health professionals.

Mandatory Requirements
- Certificate IV in Disability, Individual Support, Community Services, or equivalent in Health/Human Services.
- Mental Health First Aid Certification.
- HLTAID011 First Aid and HLTAID009 CPR.
- NDIS Worker Screening Check.
- Working with Children Check – Employer version.
- Current National Police Clearance.
- Infection Control and Manual Handling Training.
- Valid Australian Driver's Licence and comprehensively insured vehicle.
- Triple COVID-19 vaccination (including booster).

Highly Regarded
- Experience with dual diagnosis, ABI, or trauma-informed care.
- Experience supporting clients with substance use history or self-neglect.
- Familiarity with catheterisation, wound care, stoma care.
- Strong knowledge of NDIS documentation and incident reporting.
