Position Overview:
JumpIn2Life is currently seeking dedicated and compassionate Support Workers to join our team in the Illawarra region. As a Support Worker, you will play a crucial role in providing direct support and assistance to individuals with disabilities, helping them to achieve their goals and participate fully in their communities.
Key Responsibilities:
- Provide one-on-one support to individuals with disabilities in their homes, community settings, and recreational activities.
- Assist clients with daily living tasks, including personal care, meal preparation, medication management, and household chores.
- Support clients to develop independent living skills, such as budgeting, meal planning, and transportation.
- Facilitate community participation and social inclusion by accompanying clients to appointments, outings, and social events.
- Implement and monitor individualized support plans, ensuring that goals and outcomes are achieved in a person-centered manner.
- Maintain accurate and timely documentation of client progress, activities, and incidents as required.
- Communicate effectively with clients, families, and other stakeholders to ensure that support needs are met and preferences are respected.
- Collaborate with multidisciplinary teams, including therapists, case managers, and healthcare professionals, to coordinate services and achieve positive outcomes for clients.
Qualifications and Skills:
- MUST HAVE Certificate ||| in Aged Care/Home and Community Aged Care.
- Previous experience working in disability support, aged care, or a similar role is desirable.
- Excellent communication and interpersonal skills, with the ability to build rapport and establish trusting relationships with clients.
- Compassionate, patient, and empathetic approach to supporting individuals with diverse needs and backgrounds.
- Strong organisational and time management skills, with the ability to prioritise tasks and adapt to changing priorities.
- Current First Aid certificate (or willingness to obtain).
- Current National Crime Check (or willingness to obtain).
- Valid driver's license and access to a reliable vehicle.
