About the Role
As a Clinical Care Coordinator at i-HDS, you'll lead with compassion and excellence, supporting participants with complex needs across both clinical care and accommodation services. You will:
- Lead, mentor, and support a team of Support Workers across clinical and accommodation settings
- Provide hands-on clinical care and supervision, ensuring high-quality outcomes
- Develop, review, and oversee individualised care plans tailored to participant needs
- Ensure compliance with NDIS standards through regular audits, risk assessments, and clinical documentation
- Act as the primary clinical point of contact for participants, families, and allied health professionals
- Collaborate with accommodation managers to ensure holistic, person-centred support within supported living environments
About You
You're a confident, compassionate, and organised Registered Nurse who is passionate about inclusive support. You bring:
- 2–3 years post-registration experience (preferably in disability, community, or aged care settings)
- Strong working knowledge of NDIS Practice Standards and clinical governance
- Experience supporting and mentoring teams, with the ability to build capability and confidence
- A commitment to delivering person-centred care in both clinical and accommodation environments
Requirements
- Current AHPRA registration (Registered Nurse)
- Current NDIS Worker Screening Check, Police Check, and WWCC (or willingness to obtain)
- Valid Australian driver's licence
