Role Description

This is a full-time on-site role for a Specialist Support Coordinator, located in Werribee, VIC. The Specialist Support Coordinator will be responsible for providing high-quality customer service, case management, and technical support to clients. Daily tasks include coordinating individual support plans, facilitating communication between clients and service providers, and ensuring the effective implementation and monitoring of personalized support plans. Additionally, the role involves building strong relationships and providing interpersonal support to ensure individuals are empowered to achieve their goals.

Qualifications

  • Interpersonal Skills and Communication abilities
  • Experience in Case Management and Customer Service
  • Technical Support proficiency
  • Excellent organizational and time-management skills
  • Ability to work independently and as part of a multidisciplinary team
  • Knowledge of NDIS and psychosocial disability support is a plus
  • Bachelor's degree in Social Work, Psychology, or a related field
