Role Description

This is a full-time, on-site role located in Werribee South, VIC. The Community Support Worker will be responsible for providing direct support to individuals, helping them with daily activities, and fostering community engagement. Tasks include assisting with in-home support, facilitating group activities, coordinating support services, and ensuring clients' well-being. The role requires collaboration with other healthcare professionals and regular communication with clients to meet their individual needs.
