The Opportunity

Momentum 360 Partners and Co. is seeking both entry-level and experienced Positive Behaviour Support Practitioners to join our team in Yarrawonga, VIC. Positions are available to work from Shepparton, Cobram, Benalla, or Wangaratta, with the option for a hybrid model of home and office work once you are established in your role.

You will provide individualised, evidence-based support to individuals with complex needs, focusing on enhancing their quality of life and promoting positive behaviour changes. Join us in making a meaningful impact in the lives of those we support.

What you'll do

  • Conduct comprehensive assessments to understand the individual's needs, strengths, and behaviour patterns
  • Develop and implement tailored positive behaviour support plans in collaboration with the individual, their family, and the support team
  • Provide training and mentoring to support workers to ensure consistent implementation of behaviour support strategies
  • Monitor and evaluate the effectiveness of behaviour support plans, making adjustments as needed
  • Collaborate with other professionals (e.g., Occupational Therapists, Psychologists) to ensure a holistic approach to support
  • Maintain detailed case notes and contribute to the organisation's continuous improvement processes

Key Requirements

  • Tertiary qualification (minimum Bachelor's) in a relevant field, such as Psychology, Nursing(RN1), Social Work, or Occupational Therapy
  • Minimum 2 years of experience working with individuals with disabilities
  • Hold or can obtain an NDIS Practitioner ID, NDIS Workers Screening Check and Working with Children Check
  • Strong understanding of the principles of positive behaviour support and the ability to apply them in practice
  • Excellent communication and interpersonal skills, with the ability to build rapport and collaborate effectively with various stakeholders
  • Experience writing assessments and reports
