If you're driven by purpose and passionate about supporting others, this is the role for you.

Position Overview:

You will be required to:

  • Lead initiatives to obtain grants and funding resources for Heartcare
  • Evaluate local needs and available resources across health, welfare, housing, employment, education, and related services
  • Coordinate with community organisations, welfare providers, government agencies, and private sector partners to address community concerns and increase awareness of available support and initiatives
  • Identify client requirements and design, deliver, and manage targeted education, training, and support programs
  • Provide practical assistance, emotional support, and advocacy to individuals and families
  • Assist clients with access to community resources and social participation opportunities
  • Work respectfully and professionally under general guidance while managing your own time effectively
  • Maintain accurate records, exercise sound judgment, and uphold confidentiality and sensitivity in all interactions
  • Facilitate client engagement in community activities and programs, supporting participation in schemes like the National Disability Insurance Scheme (NDIS) to foster social inclusion and personal growth
  • Reflect HeartCare's values of compassion, integrity, and empowerment in all aspects of service delivery

What we need from you:

  • An associate degree, Advanced Diploma, Diploma, or equivalent qualification in Community Services, Social Work, or a related field
  • Relevant experience of at least three (3) years work experience may substitute for qualifications above
  • A current Working with Children Check and National Police Check
  • A current First Aid Certificate
  • A valid Driver's License and reliable vehicle
  • Two professional referees
  • A clear commitment to HeartCare's mission of inclusive, person-led care
  • Compassionate, patient, and empathetic with a genuine passion for caring for others.
  • Excellent communication and interpersonal skills.
  • Can work in a respectful and diligent manner.
  • Ability to work independently and as part of a team.
  • Strong problem-solving skills and the ability to handle challenging situations.

What we offer:

  • A meaningful role where you can make a genuine difference in people's lives
  • A supportive, multicultural, and inclusive team culture
  • Opportunities for ongoing professional development and training
  • A workplace that values integrity, diversity, and continuous learning
