Position Overview:
We are seeking a motivated and proactive Community Worker to join our team. This role focuses on community engagement, coordination and empowerment rather than personal care. You will play a key part in helping participants navigate services, strengthen their independence and stay socially connected.

- Experience in health, welfare, or community services (highly regarded)
- Qualifications in Community Services, Social Work, Welfare, or related fields (highly regarded)
You will be required to:
- Lead initiatives to obtain grants and funding resources for Heartcare
- Evaluate local needs and available resources across health, welfare, housing, employment, education, and related services
- Coordinate with community organisations, welfare providers, government agencies, and private sector partners to address community concerns and increase awareness of available support and initiatives
- Identify client requirements and design, deliver, and manage targeted education, training, and support programs
- Provide practical assistance, emotional support, and advocacy to individuals and families
- Assist clients with access to community resources and social participation opportunities
- Work respectfully and professionally under general guidance while managing your own time effectively
- Maintain accurate records, exercise sound judgment, and uphold confidentiality and sensitivity in all interactions
- Facilitate client engagement in community activities and programs, supporting participation in schemes like the National Disability Insurance Scheme (NDIS) to foster social inclusion and personal growth
- Reflect HeartCare's values of compassion, integrity, and empowerment in all aspects of service delivery
What we need from you:
- An associate degree, Advanced Diploma, Diploma, or equivalent qualification in Community Services, Social Work, or a related field
- Relevant experience of at least three (3) years work experience may substitute for qualifications above
- A current Working with Children Check and National Police Check
- A current First Aid Certificate
- A valid Driver's License and reliable vehicle
- Two professional referees
