We are currently seeking a passionate and experienced Memory Support Services – Advanced Lead to join our team. This newly created role offers a unique opportunity to specialise in dementia-specific services, working closely under the guidance of our Memory Support Specialist. You will play a key role in delivering high-quality dementia care to residents, their families, carers, and TLC's interdisciplinary teams.
About the Role
This is a dynamic, hands-on position focused on clinical excellence and operational efficiency. You will assist in developing and implementing person-centred care plans, with a strong emphasis on non-pharmacological strategies to support residents experiencing behavioural changes associated with dementia. Your ability to observe, document, and synthesise information will be critical in delivering evidence-based, relationship-centred recommendations.
Please note: This role requires a current driver's licence and access to a personal vehicle. Travel, including regional and overnight, may be required.
Key Responsibilities
- Conduct comprehensive assessments of residents' physical, cognitive, psychosocial, and environmental needs.
- Collaborate with residents, families, and TLC staff to develop tailored treatment plans.
- Educate staff on strategies to enhance resident independence, safety, and wellbeing.
- Maintain accurate and timely documentation in line with regulatory and organisational standards.
- Work closely with interdisciplinary teams to ensure coordinated, holistic care.
- Uphold ethical standards and resident confidentiality.
- Contribute to service improvement initiatives and program development.
- Ensure timely reporting of outcomes and efficient resource management.
- Maintain high standards of work aligned with TLC policies and relevant legislation.

- Strong interpersonal, written, and verbal communication skills.

- Ability to work autonomously and manage competing priorities.
- Proficiency in MS Office and industry-specific software.
Qualifications / Experience Required
Qualifications in Allied Health, Nursing, Aged Care, Dementia, or Mental Health (highly desirable).
Current AHPRA registration.
Experience in Australian aged care (community or residential).
Demonstrated interest in supporting individuals with mild cognitive impairment and dementia.
Knowledge of dementia types, presentations, and behavioural management strategies.
Proven clinical skills and ability to work independently.
Strong documentation and database management skills.
Leadership experience in aged or dementia care.
Understanding of the Aged Care Act 1997, Accreditation Standards, and ACFI.
