Role Statement
The Care Choices Partner plays a vital role in supporting older people and their families to navigate the care system with clarity, confidence, and compassion. Grounded in a person-centred approach, they listen closely to each client's unique needs, preferences, and goals to help them understand their options and make informed decisions about their care.
They provide practical, caring guidance through every step of the journey, from exploring services to securing the right care arrangement, while ensuring each client feels heard, supported, and respected.
Care Choices Partners also build and nurture strong relationships with care providers to ensure we can offer a comprehensive and current view of the care landscape. They bring sound judgment, knowledge, and professionalism to their work, while always striving to deliver an outstanding client experience. As part of a collaborative team, they contribute to the continuous improvement of our services and help grow My Care Journey's positive reputation across the sector.
Position Description
As the Care Choices Partner, you will be responsible for:
• Work closely with older people and their families to understand care needs, preferences and goals.
• Provide tailored advice on aged care options, including residential care, home care, respite, and NDIS supports.
• Support clients through the entire placement journey, from initial enquiry to transition, including attending tours and submitting applications or documentation on their behalf.
• Onboard new providers by gathering service details, negotiating partnership terms, and ensuring accurate system setup.
• Maintain up-to-date and accurate records of provider offerings, pricing, availability, and client interactions.
• Build and maintain strong relationships with care providers, assessors, referrers, and other key stakeholders.
• Collaborate with the internal team to improve service quality and client experience.

Qualifications, Skills & Experience
Essential
• Experience in aged care, retirement living, disability support or healthcare navigation.
• Proven experience in a client-facing role within a high-trust environment (e.g. aged care, admissions, real estate, mortgage broking, NDIS, or similar).
• Strong customer service ethos with a commitment to delivering exceptional, personalised experiences.
• Outstanding communication skills — clear, compassionate, and confident across phone, email, and in-person interactions.
• Highly organised with the ability to manage multiple tasks, timelines, and client needs simultaneously.
• A problem-solver with sound judgment and a proactive, solutions-focused mindset.
• Strong attention to detail and ability to maintain accurate records and documentation.
• Tech-confident: comfortable using CRMs, cloud-based systems, and able to adapt to new technologies with ease.
• Works well independently and collaboratively as part of a close-knit team.
Desirable
• Knowledge of the Aged Care Act, CHSP, Home Care Packages, NDIS or other relevant legislation.
• Qualifications in aged care, nursing, allied health, social work, community services or similar.
• Understanding of the residential aged care and/or retirement living sector.
• Familiarity with aged care assessment processes and placement pathways.
