At Better Days Support Services, we are dedicated to providing high-quality, person-centered support coordination to NDIS participants with a focus on mental health. We believe in fostering hope, building resilience, and empowering individuals to achieve their goals and live fulfilling lives.
Due to continued growth, we are seeking a motivated and compassionate Support Coordinator to join our dedicated team our office is based in Sebastopol, Victoria.
In this rewarding role, you will:
- Work collaboratively with participants and their families/carers to understand their goals, needs, and aspirations.
- Develop and implement individualized support plans that are tailored to each participant's unique circumstances and promote their well-being.
- Assist participants in navigating the NDIS, understanding their plans, and accessing appropriate supports and services.
- Build and maintain strong relationships with service providers, ensuring effective coordination and delivery of services.
- Monitor the progress of support plans, review outcomes, and make adjustments as needed.
- Advocate for participants to ensure their rights are upheld and their voices are heard.
- Maintain accurate and timely records in accordance with NDIS guidelines and organizational policies.
- Contribute to a positive and supportive team environment.
We are looking for someone who has:
- A genuine passion for supporting individuals with mental health challenges and a strong understanding of the recovery-oriented approach.
- Demonstrated experience in a support coordination role within the NDIS framework.
- Excellent communication, interpersonal, and relationship-building skills.
- Strong organizational and time management skills, with the ability to prioritize and manage a caseload effectively.
- A proactive and solutions-focused approach to problem-solving.
- A sound understanding of the NDIS legislation, policies, and procedures.
- Proficiency in using relevant software and technology.
- A relevant qualification in Disability, Mental Health, Social Work, or a related field (desirable).
- A current and valid Driver's License and reliable vehicle.
- A National Police Check and Working with Children Check (or willingness to obtain).
