Systems Project Manager

Care Forward is an innovative, leading provider of home care services. We are a locally owned, not for profit, Tasmanian community service provider with a focus on reablement, health and wellbeing. Our core business consists of the coordination and delivery of home support services and community care across Tasmania.

Job Tasks & Responsibilities

This Position is a 12 month Fixed-Term. Ending June 2026.

Interviews will be conducted during the advertisement period.

As a senior employee within Care Forward, you will lead and manage the implementation of Care Vision, our new client management system, and help manage changes related to the Support at Home program. You will work closely with the Technology Manager and service delivery staff to align business operations with the new system. You will also develop and implement processes to enhance service delivery efficiency and effectiveness.

Core Responsibilities


• Testing: Ensure sufficient testing of the Care Vision configuration before implementation and recommend required changes


• Business Processes: Ensure business processes are designed, tested and implemented to take advantage of the functionality offered by Care Vision and meet the requirements of the Support at Home program.


• Change Management: Manage the changes required to processes and operational structures to ensure a successful transition to Care Vision and Support at Home


• Training: Liaison with Learning and Development manager to ensure appropriate training is delivered to operational staff.


• IT Liaison: Work closely with the Technology Manager to ensure all internal staffing resources are best utilized to support operational requirements and implementation of Care Vision


• Service Delivery Liaison: Liaise with service delivery subject matter experts and management across Care Forward to identify and address inefficient processes including Intake, Scheduling, Rostering, Home Independence Stream, Care Partners, Allied Health, and Finance.


• Alignment: Ensure alignment of Care Forward processes and communication protocols across different business units including the rostering, billing and claiming functions in Care Vision to meet the requirements of Support at Home


• Organisational Change: Recommend any organisational design changes required to maximise the effectiveness and efficient delivery of services using Care Vision


• Care Vision SME: Be the Care Vision Subject Matter Expert in Care Forward.


• Delegation: Manage cross-functional teams and delegating tasks effectively

Minimum Requirements


• Experience in change management, project management and implementation of organisation wide IT systems, particularly in Client Management Systems.


• Proven leadership and project management skills.


• Understands key steps in a business improvement cycle.


• Excellent verbal, written and interpersonal communication skills – ability to communicate clearly and concisely to a diversified project team.


• Strong problem-solving skills.


• High level proficiency in Microsoft Office applications.

Highly Desired


• Experience in processes associated with consumer intake and scheduling practices in a community aged care setting.


• Experience in coordinating home care and support services community aged care setting.


• Extensive experience in a Health, Aged Care or Community Care Sector


• Ability to identify and apply new concepts into current operations


• Understanding the development of, and application of effective training
