We are growing and currently have an Endorsed Enrolled Nurse-EEN vacancy working with and supporting participants to achieve their NDIS plan goals. This is an opportunity to join the Co-operative Life supporting individuals living with disabilities and older people to lead meaningful lives by providing sustainable, person centred service with high-quality, person-centred care.

The Role Scope:

· To provide person-centred, outcome-focused care and support to TCL's clients, House Manager, Lead Support Workers, and Regional Director in line with the organisation's policies and procedures.

· Maintaining accurate and up-to-date documentation on client's progress.

· Ensuring Work Health and Safety guidelines are adhered to.

· Complying with all relevant legislation and codes of conduct.

· Maintaining current knowledge of the Aged Care and NDIS systems and practices.

· Maintaining current knowledge of local services and organisations relevant to client's needs.

· To support the Regional Director and House Manager in coordinating, planning and clinical administration of the Housing project.

· Ensuring project management methods and standards are implemented throughout the project with minimal supervision from the Regional Director.

Responsibilities and Duties:

· Coordinating and managing services to our clients (Aged Care & NDIS)

· Attending vital observation

· Administer subcutaneous injections

· Coordinate client medical appointments and medication management.

· Coordinate high-quality client services through the intake, assessment, care planning, coordination, monitoring and case management.

· Managing service provision for clients and assisting other team members as needed

· Maintaining a continuous improvement culture and being efficient in the delivery of our business strategies.

· Assisting client to strengthen their capacity to establish networks, linkages and relationships

· Providing proactive and effective client communication and building relationships and capacity with providers and their informal supports

· Using relevant communication and technology systems for reporting and administrative purposes, including creating/maintaining service.

· Positively and constructively representing the organization to external contacts at all opportunities.

· Undertaking tasks using a resourceful and creative approach.

· Suggest changes to improve the quality of your own work and make the agreed changes.

· Assisting with review and/or development, implementation and improvement of specific work practices and procedures.

· Wound Care

· Diabetes management (including insulin administration)

· Care Plans - Oversee the clinical assessment process to ensure that each client's complex care needs are identified and regularly reviewed, assessed and updated in their care plan

· Capacity Building - Advocate with and for our clients, their carers and families to enable choice and decision-making that supports independence and quality of life

· Teamwork - Contribute to a cohesive and harmonious team that respects each other's ideas, integrity and abilities. Behaving in a way that contributes to a workplace that is free of discrimination, harassment and bullying behaviour at all times

· Clinical Leadership - Complex care and specialised nursing services delivered by appropriately skilled staff under clinically focused supervision

· Allied Health Liaison - Liaising with allied health providers regarding the client's health status

· Appointments - Ensure clients attend appointments as scheduled and correct documentation is maintained.

· Medication and Medication Charts – Ensure Client Care folders and emergency information are accurate. Ensure that the GP medication summary matches the Pharmacy Webster Packs. Liaise as necessary with the pharmacy to ensure the safe and efficient delivery of medication to care recipients.

The EEN plays a vital role in supporting NDIS participants to live independently and with dignity by providing skilled and compassionate nursing care. Key responsibilities of the EEN in an NDIS context:

  • Direct Patient Care: Providing hands-on care, including assisting with personal hygiene, mobility, and other daily living activities.
  • Medication Administration: Administering medications as prescribed, ensuring accuracy and safety.
  • Monitoring and Assessment: Regularly assessing patients' conditions, reporting any changes to the RN, and documenting observations.
  • Implementing Care Plans: Following care plans developed by the RN, ensuring they are implemented effectively.
  • Communication and Collaboration: Maintaining clear communication with the RN, other healthcare professionals, and the individual's support network.
  • Maintaining a Safe Environment: Ensuring the environment is safe and conducive to the individual's needs.
  • Following NDIS Guidelines: Adhering to the policies and procedures of the NDIS and the employing organisation.
  • Scope of Practice: Working within the boundaries of their own educational preparation and demonstrated competence.

Professional Skills and Experience:

· Minimum 2 years of experience in Aged Care or Community Nurse

· Has a working knowledge of the process of through which a person identifies their goals and aspirations and person-centred plans are developed

· Understand the person-centred approach

· Understand NDIS procedures and scope

· Well-developed communication (both oral and written) skills with the capacity to prepare reports and correspondence in clear and concise language and the ability to assimilate information from other present information in a manner appropriate to the purpose and audience

· Thorough understanding and commitment to WH&S, Disability Services Standards, Home Care Common Standards and EEO principles

Professional Qualifications:

· Current and Valid AHPRA registration

· Proof of 3 doses COVID19 Vaccination requirement being met (including of the validation of the medical exemptions, if any) under the NSW Public Health Order

· Must have a current valid NSW Australian Driver's Licence

· Current NDIS Clearance

· Current First Aid Certificate

· Valid working rights within Australia
