We're looking for a vibrant and people focused Support at Home Program Support Officer to join our passionate team in Rockhampton. If you thrive on meaningful connections, love organising, and genuinely care about improving lives this is the role for you.
As a key player in our client journey, you'll be responsible for:- Connecting with clients, families, and health professionals
- Coordinating services and developing individualised care plans
- Ensuring high-quality, person-centred care delivery
- Supporting smooth onboarding and ongoing engagement
- Making every interaction matter
We're seeking someone who is both caring and capable someone who thrives on connection and brings a high level of professionalism to everything they do.
You will ideally have:
- Experience in care coordination, case management, or a similar role
- A background in aged care, disability support, or community services
- Excellent communication, time management, and problem-solving skills
- A strong ethical approach and attention to detail
- Confidence using technology and care management systems
- A current driver's licence and access to a reliable vehicle
With the new Support at Home program launching on 1st November 2025, this is an exciting time to join the sector. The program will streamline in-home aged care services across Australia, placing even greater emphasis on individualised care, consumer choice, and outcomes-based support.
As a Support at Home Program Support Officer, you will play a vital role in helping clients navigate this transition, ensuring services are tailored to their unique needs, and supporting the successful rollout of this important national reform in our local community.
Qualifications- Diploma qualified nurse highly encouraged to apply, RN / EN
- Certificate IV or Diploma in Aged Care, Disability, Community Services or similar (preferred) with minimum 2 years' experience
- Knowledge of Home Care Packages (HCP) and or NDIS (Desirable)

- Problem-Solving & Critical Thinking
- Attention to Detail & Documentation Accuracy
- Knowledge of Community/Aged Care Systems
