IDENTIFIED POSITION

THE ROLE

The Independent Living Intake Coordinator will support new and existing clients to navigate the intake process, and any external assessment required to access the Independent Living teams services. They will provide advocacy support to ensure clients receive a culturally safe plan assessment.

The position will work with the Elder Care Support Program Connector.

This role will liaise with external connections to ensure Elders and other community members are well-supported throughout the My Aged Care or NDIS assessment process.

THE IDEAL PERSON FOR THIS ROLE

Essential


• Identifies as Aboriginal and/or Torres Strait Islander


• Tertiary level training in relevant field or willingness to obtain


• Well-developed interpersonal skills and the ability to communicate effectively with people


• Have an understanding and commitment to positive Aboriginal health outcomes and Aboriginal culture


• Demonstrated ability in the planning of events


• Good record management, data collection and reporting capability


• Ability to work autonomously and be a self-starter, with a high degree of initiative and problem-solving skills


• Experience in working in a team environment that is open and transparent, with a person-centered trauma informed focus


• Current Victorian drivers' license and willingness to driver bigger vehicles including Wheelchair Bus

Desirable


• Certificate IV Lifestyle and Leisure or equivalent.


• Previous experience as a Social Support Group Coordinator or similar role would be very well regarded


• Knowledge of working to a budget and reporting on variances


• Understanding of Aged & Disability Care service area


• Relevant qualification.


• Experience in working in a not-for-profit organization, or health setting.
