Complex Case Manager

The Opportunity:
We are seeking an experienced and clinically trained Complex Case Manager to support veterans and their families through high-quality, trauma-informed case management. In this role, you will manage a caseload of clients with complex psychosocial and health needs, providing clinical expertise, coordinated support, and guidance in navigating DVA systems and external services.
You will play a key role in enhancing client wellbeing, applying the DVA Wellbeing Model, and ensuring consistent, high-standard case management practices across the Client Support Program.

The Duties:
- Provide specialised, trauma-informed clinical case management to clients with complex needs.
- Conduct intake assessments, risk assessments, and case reviews.
- Act as a primary point of contact for allocated clients, coordinating access to mental health, medical and external support services.
- Escalate high-risk client issues and work closely with Senior Clinicians and Assistant Directors.
- Build strong relationships across DVA business areas, the ADF, ex-service organisations, and community providers.
- Prepare high-level documentation, briefs and client summaries.
- Support colleagues with clinical advice, coaching and guidance for managing challenging presentations.
- Represent the program through group presentations and stakeholder engagement as required.
The Need to Haves:
- Tertiary qualifications (Bachelor or above) in Nursing, Psychology, Occupational Therapy or Social Work.
- Demonstrated experience supporting people with complex and/or co-morbid needs.
- Strong understanding of disability impacts and capacity-building support models.
- Excellent communication and stakeholder engagement skills.
- Experience in assessment, triage and working within structured case management frameworks (including telephone-based work).
Highly Desirable:
- Experience in military or veteran mental health settings.
