As an EN / Care Coordinator, you will:
- Coordinate and oversee in home care services
- Conduct care assessments, reviews, and service planning
- Collaborate with clients, families, and the care team to ensure individual needs are met
- Monitor care delivery to ensure quality, safety, and compliance
- Support carers and staff with guidance, training, and updates
We're seeking someone who is both caring and capable someone who thrives on connection and brings a high level of professionalism to everything they do.
You will ideally have:
- Previous experience in Aged Care, Community Care, or Disability Support
- Strong time management and organisational skills
- Excellent communication and interpersonal abilities
- Knowledge of care standards, assessments, and service coordination
- A current driver's licence and access to a reliable vehicle
- A genuine commitment to "Improving the quality of life for those we serve"
- A supportive, inclusive team environment
- Part-time role with the opportunity to grow into full-time

- AHPRA Registered and practicing
- Drivers Licence
- Any other Aged Care / Health Care related certificates
- Proven experience in aged care, disability, or mental health within a community setting.
- Strong understanding of Aged Care and Disability Standards, as well as consumer-directed care principles.
- Current First Aid, CPR certification, and National Police Check.
- Working With Children (WWC) check or willingness to obtain.
- A valid driver's license and access to a reliable car with comprehensive insurance.
- Knowledge of Home Care Packages (HCP) or NDIS (Desirable)
