
NDIS Service Coordinator

The NDIS Support Coordinator plays a vital role in facilitating high-quality services for individuals with disabilities. As a key member of our team, you will be responsible for documenting evidence of progress against an individual's service and support schedule.
Key Responsibilities:- Undertake comprehensive participant assessments and planning by assessing, monitoring, and reviewing the needs of the participant.
- Develop and manage participant service schedules, including service planning, confirmation, and short notice placements and cancellations.
- Manage day-to-day activities of the service, ensuring seamless execution and effective communication with stakeholders.
- Monitor participant budgets in accordance with program guidelines, ensuring financial prudence and transparency.
- Liaise with key stakeholders – both internally and externally – to ensure smooth coordination and effective service delivery.
- Research, assess, and source a variety of mainstream, community, informal, and provider options to meet the diverse needs of participants.
- Present preferred options or providers to the participant and/or plan nominee, facilitating informed decision-making.
- Negotiate services and prices for any quotable supports, ensuring value for money and optimal outcomes.
- Organize assessments as required, determining the nature and type of funding needed to support participants effectively.
- Calculate the budget for each support type and advise relevant plan managers of the breakdown of funds, promoting transparency and accountability.
- Liaise with plan managers to establish the appropriate claim categories and attribute the correct amount of funds, ensuring accurate reimbursement.
- Assist participants to prepare for their plan review by collaborating to assess achievement of goals and ensuring they obtain value for money from their plan.
This is a challenging yet rewarding role that requires strong organisational skills, effective communication, and a passion for delivering exceptional service to individuals with disabilities.
Required Skills and Qualifications- Relevant qualifications in social work, disability services, or a related field.
- Strong knowledge of NDIS policies and procedures.
- Excellent communication and interpersonal skills.
- Able to work independently and as part of a team.
- Highly organised with excellent time management skills.
- Ability to maintain confidentiality and handle sensitive information.
Applicants must possess a valid driver's license and be willing to travel occasionally. A commitment to ongoing learning and professional development is also essential for this role.
