
Aged Care Placement Assistant

What your day will look like….
You'll work closely with patients, families, and multidisciplinary teams to ensure every transition into aged care is smooth, respectful, and well-supported. Your role will involve connecting with local aged care providers to identify placement opportunities, guiding families through applications, and coordinating accurate clinical information so patients receive the care they need without unnecessary delays. Alongside this, you'll contribute to system improvements, foster strong partnerships across the health and aged care sectors, and provide compassionate support to families navigating important decisions. Each day, your work will directly shape better experiences and outcomes for patients at a critical stage of their care journey.
