About the role:

We are seeking a dedicated and passionate Community Pharmacist who has an interest in Aged Care and supporting the at risk and vulnerable members of the community. In this role you will be trained to support the initiation of clinical services in a community setting, facilitating immunisation clinics, participating in medication management review (MMR) clinical rounds, and providing aged care on-site pharmacist expertise across both metropolitan and regional locations. You will report regularly to your Team Lead and contribute to the delivery of high-quality, patient-centred care.

All travel-related expenses incurred as part of this role will be covered by Choice Aged Care.

This is a flexible opportunity, available as a permanent full-time or flexible part-time position for the successful candidate.

Key responsibilities:

  • Enthusiastically support the introduction of community based clinical services according to Choice Aged Care's mission, vision and values.
  • Prepare and administer immunisations in accordance with current clinical guidelines and protocols.
  • Develop and implement medication management plans in partnership with prescribers.
  • Provide clinical support to regional and remote communities through the delivery of vaccinations, RMMR, and HMR services.
  • Facilitate and deliver educational sessions on safe and effective medicine use for healthcare professionals, carers, and families.
  • Comply with the Aged Care Facility Clinical Governance Framework in all aspects of clinical practice.
  • Perform duties in accordance with Work Health and Safety (WHS) legislation and all relevant organisational policies and standards.

About you:

  • Passion for delivering high-quality healthcare services.
  • Current registration with the Australian Health Practitioner Regulation Agency (AHPRA).
  • Credentialed Pharmacist qualification (or willingness to complete credentialing during onboarding; estimated 60–90 hours).
  • Current immunisation certification, including CPR, First Aid, and Anaphylaxis training.
  • Experience in outreach immunisation clinics is highly regarded.
  • Strong commitment to patient care and service excellence, with a proactive and reliable approach.
  • Exceptional communication and interpersonal skills.
  • Ability to work both independently and collaboratively within a multidisciplinary team environment.
  • High attention to detail and strong documentation skills.
  • Willingness and flexibility to travel to various locations as required.
  • Appointment to this position is subject to a satisfactory National Police Clearance.
