The Nurse Navigator role aims to facilitate safe and efficient care for patients by facilitating young people accessing referrals and ongoing medical appointments that result from their comprehensive medical assessments while in youth detention

About the role

The Nurse Navigator – Youth Detention Centres is an advanced clinical practice registered nurse who demonstrates clinical and leadership knowledge, skills and abilities at advanced levels and is able to translate these into contemporary evidence-based care to practice across the system of care.

The Nurse Navigator will also support young people to attend referrals and ongoing medical appointments to prepare them for their transition out of youth detention and back into their community.

With the intention of providing holistic patient-centred care, the four key principles of the Nurse Navigator role are:

  1. coordinating patient centred care,
  2. creating partnerships,
  3. improving patient outcomes and
  4. facilitating systems improvements.

About you

Our ideal candidate will be someone who can demonstrate the following:

  • Demonstrated ability to apply effective contemporary clinical knowledge and clinical management skills in the management of nursing care.
  • Demonstrated ability to build and maintain strong relationships and to develop strong teams across the health landscape within a framework of open communication.
  • An ability and commitment to deliver clinical services collaboratively integrating the broader multidisciplinary health care team across the system in delivering high quality, patient centred care.
  • Demonstrated contemporary clinical nursing expertise with clear understanding of the requirement for evidence-based patient centred care.
  • Demonstrated ability to work autonomously and provide independent professional judgement and expertise in patient centred problem solving and clinical management.
