At Yorke and Northern Local Health Network, we believe in providing holistic care that makes a difference in people's lives. As a Transitional Care Package - Case Manager, you'll be part of a multidisciplinary team helping older people transition from hospital to home with confidence and dignity. You'll directly contribute to their rehabilitation and long-term care arrangements, providing essential support that allows them to regain their independence.

Key Responsibilities:

  • Develop and implement tailored care strategies for clients under the Transitional Care Program.

  • Collaborate with a diverse team of health professionals and external service providers.

  • Reduce hospital stays and enhance access to services for vulnerable groups.

  • Support clients through rehabilitation, ensuring successful hospital discharge and community reintegration.

  • Coordinate and evaluate a range of services, from therapy and nursing to personal care.

The successful applicant will be registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate.

  • National Police Certificate (NPC) for employment involving unsupervised contact with vulnerable groups required for this position (E.g. aged care employment or work involving vulnerable clients)
  • Immunisation Risk for this position is - Category A

SA Health Services are required to implement the Addressing vaccine preventable disease: Occupational assessment, screening and vaccination policy in the workplace.
