About this role

Catholic Homes is seeking an experienced Clinical Care Coordinator to join our team at Castledare Village located in Wilson. This is a full-time position reports directly to the Facility Manager.

Castledare Village is a well-loved medium-sized residence which is located within inner southern suburbs with easy access from Manning Road, Leach, and Albany Highway.

Key responsibilities are but not limited to:

  • Leading a team of staff, including Care Workers, Lifestyle staff members and Hospitality (Catering, Cleaning and Laundry).
  • Providing additional training, education and toolbox to staff in response to identified gaps and as per the direction of the Facility Manager.
  • Reviews Assessments, Forms and Charts to develop and update Care plans and Progress Notes.
  • Following safe work practices for direct care in line with Catholic Homes Incorporated policies and procedures and relevant legislation
  • Assisting the clinical team with adhering to Catholic Homes' policies and procedures relating to the clinical care of Residents
  • Supporting and advocating the rights of Residents to express their needs, choices and preferences in living their lives
  • Supporting the Facility Manager to maintain Regulatory Compliance

To be successful in this role you will have/be

  • Current registration as an Enrolled Nurse or Registered Nurse (or similar) with the Australian Health Practitioner's Regulation Agency (AHPRAH) is required.
  • Two to three 3 years' experience in a similar role in the health industry, preferably in Aged Care Sector.
  • Experience in AN-ACC assessment and care planning documentation
  • A good knowledge and understanding of the AN-ACC funding system.
  • Knowledge of business rules for AN-ACC and Aged Care Standards and Accreditation requirements.
  • Knowledge and understanding of continuous quality improvement and risk management principles.
